Client Service Agreement

A copy of this agreement will be located outside of salon and must be signed before you enter the space.

As a client of Nufaces Int,

I understand that I must agree to these guidelines before services are rendered.

 I agree to the following:

  •  I will wear a face mask during my appointment. (If you do not have a mask, one will be provided for you and a PPE fee will be charged to your total)

  • I do not have a fever, cough or any COVID related symptoms

 I will reschedule my appointment if:

  1.  I am not feeling well for any reason.

  2.  I have been exposed to anyone that has tested positive for the coronavirus in the past 14 days.

  3.  I have any reason to believe that I have or had the coronavirus within the last 14 days.

  •  I will allow a check of my temperature with a touchless Thermometer upon arriving to the salon,

  • I understand that if my temperature is over 99 degrees my appointment will be rescheduled. 

  •  I acknowledge that I am in a public space, and preventative measures to reduce the spread of COVID-19 has been put in place. However, I do not hold the salon owner or its stylists responsible, nor can they guarantee that you will not become infected.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
